I'm happy you want to come back to the church! Unless you've been excommunicated, it's not necessary to be re-baptized, even if you've been baptized in another religion. Your previous baptism in the Church of Jesus Christ of Latter-day Saints still remains in effect.
Hi friend. I just learned that in some cases rebaptism may in fact be possible/necessary for those who have joined other churches. You ought to chat with your bishop about the possibility. Best of luck!
To come back to the church, I suggest you look up the Mormon congregation nearest to your house online and chat with the local Bishop. Perhaps the missionaries could also help you return.
